About the Patient Centered Medical Home Collaboration

We recognize that communities benefit from a common strategy across employed and independent practices. MaineHealth and the MMC Physician-Hospital Organization are partnering to provide leadership, training and technical assistance to support practice transformation to a Patient Centered Medical Home for all practices in the Maine Region. Read the MaineHealth PCMH Consensus Definition and Joint Principles

Our intention is to partner with each practice to assess strengths and opportunities for transformation based on the NCQA’s 2011 Patient Centered Medical Home Standards and Guidelines. Ongoing communication with practical and timely information will be available on a regular basis via the MMC PHO weekly e-news, and on the MaineHealth PCMH website (which you are currently on).

Our Internal PCMH Team:

Jeff Aalberg, MD
Dr. Aalberg received his BS from Rensselaer Polytechnic Institute, his MS from the University of Oregon, and his MD from the University of Massachusetts.  After years in private practice, Dr. Aalberg joined the Faculty of the Maine Medical Center Family Medicine Residency Program.  He served as Medical Director and Associate Chief of Family Medicine until he entered his current role as MMC Physician-Hospital Organization Senior Medical Director.  He has also served the MaineHealth system as a Medical Director with concentrations in infection control, orthopedic registries and pharmacy.  

Cynthia Cartwright, MT RN MSEd
Cynthia is a Program Manager in Clinical Integration at MaineHealth. She has run Learning Collaboratives and coached primary care practices as they integrated mental health services since 2005. She completed the Dartmouth Institute’s Clinical Microsystems Coach the Coach Program in 2006. Before joining MaineHealth, Cynthia worked for 12 years as an education specialist at the Family Medicine Residency Program at Maine Medical Center.  She has published and presented on a variety of topics including program development, process improvement, depression in primary care, and mental health integration.

Shawn Hutchinson, MS
Shawn coordinates the PCMH program within Clinical Integration at MaineHealth. His education and experience include communications, project management and marketing in both business and healthcare environments. As part of the PCMH team at MaineHealth, he manages the PCMH website, plans the collaborative with the internal team, manages the Collaborative Learning and Coach Development events and acts as the communication liaison to the public and participating teams and coaches.

Valerie Jackson
Valerie joined the PHO in 2010, bringing with her more than 20 years of experience in business and industry.  She brings with her a diverse background in project management, process improvement, customer service and IT.   As a Dartmouth Institute-trained Clinical Microsystems Coach and certified TeamSTEPPS trainer, Valerie utilizes a variety of tools to help interdisciplinary teams create sustainable systems, improve outcomes and increase patient/staff satisfaction.  She has coached over twenty teams, including inpatient, outpatient primary care, and outpatient specialty groups. Valerie is an experienced facilitator and trainer, and has developed and conducted training locally, regionally, and nationally.  In addition to leading the PHO Practice Improvement Team, Valerie is a member of the MaineHealth/ PHO Patient Centered Medical Home Collaborative Team and Faculty.

Cindy Richards, MPH
Cindy has over twenty-five years of experience in the health field with an extensive background in the planning, implementation and evaluation of broad based public health and clinical initiatives. At MaineHealth she serves at the Program Director for Clinical Integration. Prior to joining the organization, Cindy served as the Director of Prevention and CardioPulmonary Rehabilitation at St. Mary's Regional Medical Center and previously held the position of Director of Health and Productivity Programs at UnumProvident where her programs earned national recognition twice earning the C. Everett Koop Health Award as well as the Center for Disease Control HIV/AIDS Leadership Award. 

Michael Yunes, MPH
Michael is the Manager of Quality for Maine Medical Partners (MMP) where he has served in his current role for four years. Before coming to Maine Medical Partners Michael worked as a professional project manager running large scale clinical trials for the pharmaceutical industry. In his current role Michael leads a team that monitors compliance to external quality programs, continuously improves their robust internal quality program, and responds to practice’s improvement needs. He also provides leadership and oversight on quality measurement development, data collection, data management and data analysis for all MMP practices covering more than 300 physicians.  Michael is a trained Clinical Microsystems coach through the Dartmouth Institute and continues to coach front line teams in process improvement.  Michael actively develops internal training programs for the spread of quality improvement tools and development of quality improvement coaches within MMP and enjoys serving as faculty for system level quality training initiatives.
