The goal of the PCMH Learning Collaborative is to provide the knowledge, skills and tools necessary to optimize care through a patient centered, coordinated approach. We teach care teams to work together and look at their work through microsystems and process oriented lenses in order to achieve high level patient care, and a high level of patient and staff satisfaction, in addition to creating an efficient practice.
Participating practice teams will receive support as they move through the transformation process and will have access to improvement coaching and technical assistance. For a list of change and improvement tools used throughout the collaborative learning sessions click here.
We have been fortunate to work with many enthusiastic and high performing teams throughout the 4 waves (so far) of our learning collaborative.
The general format we follow is 2 full or half day sessions of learning microsystems theory and tools and as much time for teams to practice real-time as possible.
The final (3rd) session includes some didactic teaching, but the highlight is that each team creates a presentation complete with posters, creative designs, prizes (and even lobster in one case), to showcase one of their greatest successes or learning from becoming a medical home.
