I Want to be a Living Donor!
If you have decided you want to take the next step toward being a living kidney donor, here are some documents that you will be required to review and/or complete before you can move forward:
Living Donor Handbook - An overview of being a living kidney donor from initial contact through post-surgery follow-up. Please read this carefully! It should answer most of your questions, but if you need more information, don't hesitate to call us.
Living Donor Education Class - Roxanne Taylor, RN, MSN, CCTC, CPT - A slide presentation and discussion of more aspects of living donation.
Steps to Living Kidney Donation - This document outlines all the information you will need to provide and the documents you will need to complete to begin the process of donating a kidney. Please follow these directions exactly. All documents must be completed before you can start the evaluation process.
Consent to Undergo Evaluation for Living Kidney Donation - Signing this document will give us permission to begin evaluating you as a potential living kidney donor.
Maine Medical Center Pre-Registration Form - For living donor evaluation to go forward, we will need the information on this form to register you as a patient at the Maine Medical Center. Please fill out this form completely. We will not be able to arrange for your evaluation if this form is incomplete.
Health History Form - In order to make sure you are healthy enough to donate a kidney, we need information about you and your health history. Please be as complete and accurate as you can.
Medical Records Release Form - We will need to get the last three years of your medical records from your primary care physician and any other health care providers that you may see on a regular basis. Instructions for completing this form are found in the Steps to Living Kidney Donation.
