MaineHealth wants all of its patients and employees to have a voice in their care.  Advance care planning lets the patient state his/ her care wishes if there is a health crisis. The following information will help you get started.

What is advance care planning?

Sometimes we can become hurt or suddenly sick and we are not able to make our own health choices.  Imagine if you were in a car accident and unaware of what was going on. Who would make healthcare choices for you?  Does that person know your wishes for care?

Planning ahead for the unexpected is normal and important.  Advance care planning is a way to tell your loved ones and doctor what kind of care you want if something happens. 

Talking about your choices while you are healthy makes it easier for family / loved ones during a health crisis.

You do not need to hire anyone to help you with advance care planning. The process is free, but it takes some time.
