Advance Directives

What is an Advance Directive?

An advance directive includes:

  • a written statement of your wishes for the future health care
  • a written choice of a healthcare agent to make healthcare decisions for you (also known as a durable health-care power of attorney, health care proxy or surrogate)

How can I make changes to an advance directive?

You can change or cancel your advance directive document at anytime. Simply complete a new form and replace all of the old advance directive copies with the new advance directive. You need to replace the forms at all locations such as your  family members, doctor’s office, local hospital and healthcare agent.  Take time to tell your  loved ones about your new wishes – this is very important.
